Our Mission is to deliver individualised culturally safe support for NDIS participants, Indigenous or non- Indigenous, with the purpose of achieving short and long term goals by working in partnerships with participants, their families and community. Positive Transition- Each One Teach One also take pride in creating a safe space for all involved within the organisation.  

Supported Independent Living

> Positive Transition- Each One Teach One offers Supported Independent living to assist participants to achieve their goals. We tailor these services around your specific needs and interest. These Supports can occur in home and out in the community. 

SIL Accommodation

> Positive Transition- Each One Teach One provides Emergency Respite, Short and Medium team accommodation for participants who are looking to increase their independent living skills in a supported environment. 

Community Access

If you want to stay connected with or start to become more active in your community, we can support you. Our social and community participation supports are flexible and completely tailored to you and your goals, and can help you build your self-esteem, enhance your social life and improve your general wellbeing.

Assistance with Daily Living

> Positive Transition- Each One Teach One offers assistance with personal activities and household task such as cleaning and meal preparation.

> Transport to community activities, medical appointments and other transportation needs.

> Personal care such as personal hygiene, bathing, dressing, continence support an other task as requested by you. 

> Improving health and wellbeing through exercise and physiology and education.
